Getting Ready for 2010-2011
Below you will find helpful information to assist you and your family as you prepare for the upcoming year. You can download, review and print as needed:
Medical and Physical Requirements
All students who will be entering
Early School, Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grade - as well as ALL new students - will
need a physical to begin the school year. A signed doctor form - and
immunization form if any vaccinations are required - must be turned in
to the School's Health Office at the beginning of the school year. Click on the following to download:
- Physical form to be completed and signed by your child's physician
- Immunization form to be updated as immunizations are required (to see a list of required immunizations from the State of Wisconsin, click here)
- Medication form
must be filled out and returned to the School's health office if you
child takes any type of medication during the school day. If you have
